ABSTRACT

CT screening has markedly reduced the lung cancer mortality in high-risk population and increased the detection of early-stage pulmonary neoplasms, including multiple pulmonary nodules, especially those with a ground-glass appearance on CT. Multiple primary lung cancer (MPLC) constitutes a specific subtype of lung cancer with indolent biological behaviors, which is predominantly early-stage adenocarcinoma. Although MPLC progresses slowly with rare lymphatic metastasis, existence of synchronous lesions and distributed location of these nodules still pose difficulty for the management of such patients. One single operation is usually insufficient to eradicate all neoplastic lesions, whereas repeated surgical procedures bring about another dilemma: whether clinical benefits of surgical treatment outweigh loss of pulmonary function following multiple operations. Therefore, despite the anxiety for treatment among MPLC patients, whether and how to treat the patient should be assessed meticulously. Currently there is a heated discussion upon the timing of clinical intervention, operation mode and the application of local therapy in MPLC. Based on clinical experience of our multiple disciplinary team, we have summarized and commented on the evaluation, surgical treatment, non-surgical local treatment, targeted therapy and immunotherapy of MPLC in this article to provide further insight into this field.

ABSTRACT

Objective:To build and optimize the academic performance evaluation system and process of medical colleges, and to provide reference and support for the construction of "double first-class" medical colleges.Methods:Based on CIPP model (context, evaluation, input evaluation, process evaluation and product evaluation), this paper studied the evaluation method of "double first-class" university academic performance analysis, constructed the evaluation model of medical colleges academic performance from four elements of reflecting the background, input, process and results of academic research activities, established the evaluation process, and selected 10 universities for empirical research.Results:The comprehensive weight was obtained by entropy weight method and expert scoring method. The relative proximity between each evaluation object and the optimal scheme was calculated by technique for order preference by similarity to an ideal solution (TOPSIS) method, and the comprehensive calculation value of academic performance level of each university was obtained.Conclusion:The construction of academic performance evaluation system of universities based on CIPP model and entropy weight TOPSIS method is an effective discussion and expanding research on academic evaluation of colleges, and can provide references for the analysis of academic performance with medical characteristics.

ABSTRACT

Objective To investigate the pathogenesis of superficial mycosis among naval trainees, and observe the efficacy of a novel antifungal drug. Methods A questionnaire survey was conducted on the onset, medication and recurrence of superficial fungal infection among the trainees from January, 2020 to July, 2020. At the same time, the new antifungal drug sulconazole nitrate spray was provided for treatment and the drug efficacy was observed. Results The participants generally lacked understanding and attention to superficial fungal infections. The incidence rate of superficial fungal infection was 52%, of which 76.2% of patients had recurrence of superficial fungal infection. The sulconazole nitrate spray showed great effect against these infections. Conclusion The trainees should understand the causes of superficial fungal infection through health education and seek medical treatment and medication in time. The cure rate of superficial fungal infections could only be improved through the collaborative management of the school, hospital, and trainees to reduce the impact of these infections on naval trainees’ work and life.

ABSTRACT

Metabonomics is an important component of systems biology.It reveals the essential metabolic characteristics of the activities of organisms using qualitative and quantitative detection and analysis of the dynamics of all endogenous low-molecular-weight metabolites within an organism before and after stimulation such as pathophysiological stimuli or genetic modification.Therefore, it provides insight into the pathogenesis, early diagnosis, treatment and prognosis of diseases.Metabonomics relies on chromatography, mass spectrometry, nuclear magnetic resonance spectroscopy and other analytical chemistry techniques to obtain data.The examination specimens are usually body fluids including blood, tear, aqueous humor and tissues such as trabecular meshwork, vitreous body, retina, etc.Glaucoma is an optic nerve disease characterized by optic disc damage and visual field defect.Previous animal and human studies have provided some preliminary results on metabolites associated with glaucoma.Metabonomics, genomics, transcriptomics, and proteomics constitute a bioinformatics system.Joint multi-omics research will be the direction of future development.On the basis of an overview of metabonomics, this paper reviewed the research progress of metabonomics in glaucoma based on different tissues and body fluid specimens.

ABSTRACT

AIM:To investigate the neuroprotective effect of 17β-estradiol(E2)on retina light damage in BALB/c and C57BL/6 mice and provide experimental data for the successful construction of a research model for E2 against retinal light damage.METHODS:Totally 40~45 adult female BALB/c or C57BL/6 mice were divided into six groups, 6 for each group: normal control, ovariectomized control, ovariectomized light(mice were stimulated with continuous white light at 10000 lx for 4, 8, 12, 16, and 24h after 14d of ovariectomy), intravitreal administration sham operation, saline and E2 pre-treatment groups(2μL saline or 10-5mol/L E2 were intravitreal injected respectively after 14d of ovariectomy operation and 24h of dark adaptation). The morphological and functional changes of the retina were detected by paraffin section HE staining, TUNEL staining and electroretinogram.RESULTS:In the ovariectomized light group, the thickness of the inner/outer nuclear layer decreased significantly from the 4h stimulation of 10000 lx white light group. Intravitreal administration of E2 significantly inhibited the apoptosis of retinal cells in the two strains of mice(P<0.01)and the decrease of amplitudes of a- and b-waves in max-ERG of C57BL/6 mice(P<0.05).CONCLUSION:The light loss sensitivity of two strains of mice was different under the same light stimulation. E2 had a protective effect on both morphology and function of the retina in BALB/c mice, and had a significant protective effect on retina function in C57BL/6 mice.

ABSTRACT

Single-door laminoplasty has been widely used in the treatment of multisegment cervical myelopathy, with the clinical advantages of decompression of the spinal cord, relieving preoperative neurological symptoms or signs, and maintaining cervical mobility. However, in clinical work, patients with limited cervical spine activity after single open door laminoplasty are often encountered, and the direct contact with the adjacent vertebral arch can be observed in the postoperative X-ray of the anterior and lateral cervical spine, which is called the adjacent vertebral arch bone impact, which is one of the important causes of the limited cervical spine movement. In recent years, there have been many reports on the prevention of bone impact, although the short-term clinical effect is significant, but long-term clinical efficacy to be further study, and the cause and the pathogenesis of bone impact is no consensus, this paper on the surgery of adjacent vertebral arch impact epidemiology, biomechanics, clinical performance, surgical effect and improvement.

ABSTRACT

Objective:To investigate the prevalence of hyperuricemia (HUA), patients′ awareness of HUA, and related factors among elderly in community.Methods:The health check-up data of 6 897 residents aged over 65 years in Gumei Community of Shanghai were collected from January 2019 to January 2020. There were 1 156 subject with increased serum uric acid levels (HUA group) and 5 741 with normal uric acid levels (non-HUA group). The differences of clinical indicators between HUA group and non-HUA group were analyzed and the risk factors of HUA was determined by multivariate logistic stepwise regression. A questionnaire survey on the knowledge of HUA and the adoption of relevant health behaviors was conducted among HUA patients.Results:The overall prevalence was 16.8% (1 156/6 897) in this population. The prevalence in males was significantly higher than that in females[26.4%(842/3 195) vs. 8.5%(314/3 702), P<0.001); and the prevalence in females increased with age (χ 2=7.56, P=0.023). Body mass index(BMI), waist circumference(WC), total cholesterol(TC), triglyceride(TG), serum creatinine, alanine aminotransferase(ALT) and albumin/urine creatinine ratio(UACR) in the HUA group were significantly higher than those in the non-HUA group, while estimated glomerular filtration rate(eGFR) and high-density lipoprotein(HDL) were lower than those in the non-HUA group (all P<0.01). The prevalence of hypertension, decreased renal function, overweight or obesity, abdominal obesity, fatty liver and renal cyst in HUA group was significantly higher than that in non-HUA group (all P<0.01). High values of TG, low-density lipoprotein(LDL), WC, hypertension and fatty liver were risk factors for HUA( OR=1.14, 1.20, 1.03, 1.43, 2.19; P<0.01); while female gender, eGFR, HDL and glycosylated hemoglobin A1c(HbA1c) were protective factors for HUA( OR=0.32, 0.94, 0.65, 0.78; P<0.01). The questionnaire survey was conducted among 1 090 HUA patients, and the results showed that 73.2% (798 cases) were aware of the disease after the health check-up results released; only 30.9% (337 cases) knew the diagnostic criteria of HUA, 21.1% (230 cases) knew that HUA needed life-long follow-up care, 56.3% (614 cases), 49.2% (536 cases) and 47.9% (522 cases) thought that HUA should eat less seafood, broth and soya bean products, 17.0%(185 cases) were atcohol drinker and the awareness rates of above questions in patients with gout were higher than those in patients without gout ( P<0.05). Conclusion:The study shows that the prevalence of HUA among the elderly in Gumei community of Shanghai is high, and the HUA related knowledge levels and health behavior performance are not ideal, especially for HUA patients without gout, therefore health education should be strengthened for elderly residents in the community.

ABSTRACT

Objective:To establish and verify the prediction model of benign or malignant of solitary pulmonary nodules (SPNs≤20 mm) based on artificial intelligence.Methods:Totally 338 SPNs (≤20 mm) from 279 patients, confirmed by operation and pathology, were selected in Zhongshan Hospital Xiamen University from November 2018 to May 2020. Clinical data (age, gender, smoking history, individual and family history of malignancy), image features (maximum diameter, minimum diameter, solid proportion, volume, lobulation sign, burr sign, vacuole sign, cavity sign, pleural indentation sign, and radiomic features (maximum CT value, minimum CT value, average CT value, median CT value, CT value standard deviation, skewness, peak, energy, entropy) were analyzed retrospectively. All the data of patients were randomly divided into training set (271 SPNs) and test set (67 SPNs). In the training set, the clinical, image features and radiomic features were first selected by the least absolute shrinkage and selection operator (LASSO) regression, then the independent risk factors of SPN (≤20 mm) were screened out by multi-variate logistic regression analysis, and the nomogram prediction models were constructed. Finally, the data of test set were used to verify the prediction model by the ROC curve and calibration curve (CC).Results:In the training set of 271 SPNs, 81 SPNs were benign and 190 malignant. After analysis of LASSO regression and multi-factor logistics regression, the independent predictors of benign or malignant SPN were age, gender, largest diameter, vacuole sign and solid proportion. The prediction model was P=e x/(1+e x), x=-2.583+0.027×age+1.519×gender+0.127×maximum diameter-2.132×solid proportion+1.720×vacuole sign. The results of the model showed that the area under curve (AUC) of ROC was 0.850, and the sensitivity was 73.7%, specificity was 82.7% and accuracy was 82.3%. In the test set of 67 SPNs, 22 SPNs were benign and 45 malignant. The results showed that the AUC of ROC was 0.882, and the sensitivity was 82.2%, specificity was 81.8% and accuracy was 85.1%. The calibration nomogram of prediction model showed that CC from training set or test set well coincided with its individual ideal curve ( Ptraining=0.688, Ptest=0.618). Conclusion:Prediction model of benign or malignant SPN ≤20 mm is established based on AI; it can obtain the prediction probability and has good diagnostic efficiency.

ABSTRACT

Objective:To explore the clinical application and long-term safety of hydroxychloroquine sulfate (HCQ) in the treatment of rheumatic diseases.Methods:A multi-center cross-sectional study was conducted between August 2017 and August 2018 in a random sample of eleven medical institutions of rheumatology and immunology in China. Patients who took HCQ for more than 3 months were enrolled into this study. The cumulative dose and long-term side effects of HCQ were recorded. The changes of laboratory indexes before and after treatment with HCQ were analyzed. Categorical variables were presented with counts and proportions, and evaluated by Chi-square test. Continuous parametric data were presented as Mean±standard deviation, and evaluated by Student's t test or Mann-Whitney U test. P-values less than 0.05 were considered statistically significant. Results:A total of 886 patients with rheumatic diseases were enrolled into this study, including 505 cases with systemic lupus erythematosus (57.0%), 210 cases with rheumatoid arthritis (23.7%), 80 cases with Sj?gren's syndrome (9.0%), 57 cases with undifferentiated connective tissue disease (6.4%), 12 cases of systemic vasculitis (1.4%), 10 cases of mixed connective tissue disease (1.1%), 7 cases of myositis (0.8%) and 5 cases with systemic sclerosis (0.6%). The most common long-term side effects of HCQ was skin or mucous lesions (12.4%) and vision problems (8.0%). Other adverse reactions included problems of digestive system (3.0%), nervous system (2.1%), musculoskeletal system (1.1%) and cardiovascular system (0.9%). 140 cases (15.8%) had stopped taking HCQ during the treatment. More than half of them decided to stop taking medicine by themselves. Fifty-four patients (6.1%) stopped using HCQ due to side effects while 24 of them took it again, and another 12 patients (1.4%) stopped the drug due to remission of illness. Patients were divided into three groups according to the cumulative dose of HCQ: less than 500 g, 500-1 000 g and more than 1 000 g respectively. There was significant difference in the incidence of long-term side effects among the three groups ( χ2=6.382, P=0.041). The last group (more than 1 000 g) suffered the highest incidence of long-term adverse reactions (37.1%). No severe adverse drug reactions were observed in this study. Conclusion:Hydroxychloroquine is widely used in the treatment of rheumatic diseases. The incidence of long-term side effects is 20.4%, is 6.1% lead to drug withdrawal, which are especially related to the cumulative doses. It should be adjusted properly according to the clinical application.

ABSTRACT

Objective:To observe the efficacy of addition and subtraction therapy of Danshenyin combined with Wendantang in the treatment to stable angina pectoris (SAP) with stagnation of phlegm and blood stasis, and to explore its protection mechanism for myocardial ischemia. Method:One hundred and thirty-two patients were randomly divided into control group and observation group equally. Finished the 63 cases study both in control group and observation group after dropout, loss of follow-up and withdrawal. Patients in control group and observation group got antianginal drugs and the treatment of drug therapy to control the risk factors. All patients were treated with anti-angina drugs and risk factors control drugs. Patients in control group got Danlou Tablets by oral administration, 5 tablets/time, 3 times/day. Patients in observation group got dispensing decoction pieces of Danshenyin and Wendantang 1 dose/day. The treatment continued for 3 months in both groups. Scores of angina attack were graded every week. Before and after treatment, electrocardiogram treadmill exercise test was made to evaluate myocardial ischemia of coronary heart disease, and scores of phlegm stasis block syndrome and Seattle Angina questionnaire (SAQ) were graded for TCM symptoms and quality of life. Levels of hemorheology index, interleukin-6 (IL-6), tumor necrosis factor-<italic>α</italic> (TNF-<italic>α</italic>), intercellular adhesion molecule-1 (ICAM-1), Cystatin C (CysC), homocysteine (Hcy), ischemic modified albumin (IMA) and macrophage migration inhibitory factor were detected. In addition, safety was evaluated. Result:After treatment, scores of times, duration, degree of angina pectoris, nitroglycerin dosage of angina pectoris and nitroglycerin dosage in the observation group were lower than those in the control group (<italic>P</italic><0.01). Total exercise time, duration of ST depression for 1.0 mm, occurrence time of stable angina pectoris, metabolic equivalent and scores of Duke in the observation group were more than those in the control group (<italic>P</italic><0.01). Score of stagnation of phlegm and blood stasis in the observation group was lower than that in the control group (<italic>P</italic><0.01), while score of SAQ was higher than that in the control group (<italic>P</italic><0.01). Levels of IL-6, TNF-<italic>α</italic>, ICAM-1, CysC, IMA, Hcy, MIF, whole blood viscosity (low cut, high cut), whole blood reducing viscosity, plasma viscosity, platelet aggregation rate and fibrinogen (FIB) in the observation group were lower than those in the control group (<italic>P</italic><0.01). Effect of angina pectoris in observation group was superior to that in control group (<italic>Z</italic>=2.091, <italic>P</italic><0.01). No adverse reactions related to Danshenyin combined with Wendantang were found. Conclusion:Addition and subtraction therapy of Danshenyin combined with Wendantang based on the routine western medicine treatment can control the attack of angina pectoris, relieve the symptoms of phlegm and stasis block syndrome, and improve the quality of life for patients with SAP, showing superior clinical efficacy and safety. In addition, it can improve the hemorheology of patients, inhibit the inflammatory reaction, reduce the stenosis or obstruction of lumen in order to improve the degree of myocardial ischemia.

Objective@#To investigate the application of suprapubic lipectomy with a "Ω" incision to removal of the prepubic fat pad for the management of buried penis in obese adult patients.@*METHODS@#We retrospectively analyzed the clinical data on 20 obese adult patients with buried penis treated by suprapubic lipectomy via a "Ω" incision between August 2016 and September 2019.@*RESULTS@#The operations were successfully completed in all the cases, with a mean operation time of 3.7 ± 0.6 hours and an average hospital stay of 8.3 ± 3.3 days. There were no such severe surgery-related complications as hematoma, urethral injury, or fat embolism in any of the cases. Fat liquefaction-related superficial wound infection developed in 1 patient postoperatively, which was cured by combined topical and systemic antibiotic therapy. A 3-month follow-up showed a 95% satisfaction of the patients with the postoperative appearance of the penis and suprapubic incision, but no complications such as ED, abnormal penile sensation, or penile retraction.@*CONCLUSIONS@#Suprapubic lipectomy with a "Ω" incision to remove the prepubic fat pad is an effective surgical approach to the management of buried penis in obese adult males, which is an anatomy-based surgical correction and has the advantages of slight injury, rapid recovery and few complications./.

ABSTRACT

In this study, ultra-performance liquid chromatography coupled with quadrupole time-of-flight tandem mass spectrometry(UPLC-Q-TOF-MS)-based liver metabolomics approach was used to explore the mechanism of "Trichosanthis Fructus-Allii Macrostemonis Bulbus" in improving atherosclerosis(AS) of mice with apolipoprotein E gene knockout(ApoE~(-/-)). AS mouse model was induced by high-fat diet. The pathological and biochemical indexes such as the histopathological changes, body weight, liver weight, blood lipid level and inflammatory factors in the liver of mice were determined. The metabolic profiling of mice liver samples was performed with UPLC-Q-TOF-MS. Multiple statistical analysis methods including partial least squares discriminant analysis(PLS-DA) and orthogonal partial least squares discriminant analysis(OPLS-DA) were employed to screen and identify biomarkers. The levels of related enzymes including LCAT, sPLA2, EPT1 and ACER1 were detected. The results showed that "Trichosanthis Fructus-Allii Macrostemonis Bulbus" significantly reduced the areas of aortic plaque and fat vacuoles of liver in AS mice and decreased the accumulation of lipid droplets and liver coefficient. "Trichosanthis Fructus-Allii Macrostemonis Bulbus" also regulated the levels of blood lipid and inflammatory injury in the liver. The metabolites of the control group, the model group and the "Trichosanthis Fructus-Allii Macrostemonis Bulbus" group could be distinguished significantly. Fifteen potential biomarkers related to AS were discovered and preliminarily identified, seven of which could be regulated by "Trichosanthis Fructus-Allii Macrostemonis Bulbus" in a trend of returning to normal. Metabolic pathway analysis screened out two major metabolic pathways. "Trichosanthis Fructus-Allii Macrostemonis Bulbus" obviously regulated the levels of LCAT, sPLA2, EPT1 and ACER1. It was inferred that "Trichosanthis Fructus-Allii Macrostemonis Bulbus" could play a major role in AS treatment by regulating glycerophospholipid and sphingolipid metabolism disorders in the liver, with the mechanism probably relating to the intervention of the expression of LCAT, sPLA2, EPT1 and ACER1.

ABSTRACT

OBJECTIVE@#To detect the serum level of soluble chemokines CXCL9 and CXCL10 in patients with rheumatoid arthritis (RA), and to analyze their correlation with bone erosion, as well as the clinical significance in RA.@*METHODS@#In the study, 105 cases of RA patients, 90 osteoarthritis (OA) patients and 25 healthy controls in Peking University People's Hospital were included. All the clinical information of the patients was collected, and the serum CXCL9 and CXCL10 levels of both patients and healthy controls were measured by enzyme-linked immune sorbent assay (ELISA). CXCL9 and CXCL10 levels among different groups were compared. The correlation between serum levels with clinical/laboratory parameters and the occurrence of bone erosion in RA were analyzed. Independent sample t test, Chi square test, Mann-Whitney U test, Spearman's rank correlation and Logistic regression were used for statistical analysis.@*RESULTS@#The levels of CXCL9 and CXCL10 were significantly higher in the RA patients [250.02 (126.98, 484.29) ng/L, 108.43 (55.16, 197.17) ng/L] than in the OA patients [165.05 (75.89, 266.37) ng/L, 69.00 (33.25, 104.74) ng/L] and the health controls [79.47 (38.22, 140.63) ng/L, 55.44 (18.76, 95.86) ng/L] (all P < 0.01). Spearman's correlation analysis showed that the level of serum CXCL9 was positively correlated with swollen joints (SJC), rheumatoid factor (RF) and disease activity score 28 (DAS28) (r=0.302, 0.285, 0.289; P=0.009, 0.015, 0.013). The level of serum CXCL10 was positively correlated with tender joints (TJC), SJC, C-reactive protein (CRP), immunoglobulin (Ig) A, IgM, RF, anti-cyclic citrullinated peptide antibody (ACPA), and DAS28 (r=0.339, 0.402, 0.269, 0.266, 0.345, 0.570, 0.540, 0.364; P=0.010, 0.002, 0.043, 0.045, 0.009, < 0.001, < 0.001, 0.006). Serum CXCL9 and CXCL10 levels in the RA patients with bone erosion were extremely higher than those without bone erosion [306.84 (234.02, 460.55) ng/L vs. 149.90 (75.88, 257.72) ng/L, 153.74 (89.50, 209.59) ng/L vs. 54.53 (26.30, 83.69) ng/L, respectively] (all P < 0.01). Logistic regression analysis showed that disease duration, DAS28 and serum level of CXCL9 were correlated with bone erosion in the RA patients (P < 0.05).@*CONCLUSION@#Serum levels of CXCL9 and CXCL10 were remarkably elevated in patients with RA, and correlated with disease activities and occurrence of bone erosion. Chemokines CXCL9 and CXCL10 might be involved in the pathogenesis and bone destruction in RA.

ABSTRACT

Background@#A novel coronavirus (2019-nCoV) causing an outbreak of pneumonia in Wuhan, Hubei province of China was isolated in January 2020. This study aims to investigate its epidemiological history, and analyzed the clinical characteristics, treatment regimens and prognosis of patients infected with 2019-nCoV during this outbreak.@*Methods@#Clinical data from 137 2019-nCoV-infected patients admitted to the respiratory departments of nine tertiary hospitals in Hubei province from December 30, 2019 to January 24, 2020 were collected, including general status, clinical manifestations, laboratory test results, imaging characteristics, and treatment regimens.@*Results@#None of the 137 patients (61 males, 76 females, aged 20-83 years, mean age 55 ± 16 years) had a definite history of exposure to Huanan Seafood Wholesale Market. Major initial symptoms included fever (112/137, 81.8%), coughing (66/137, 48.2%), and muscle pain or fatigue (44/137, 32.1%), with other, less typical initial symptoms observed at low frequency, including heart palpitations, diarrhea, and headache. Nearly 80% of the patients had normal or decreased white blood cell counts, and 72.3% (99/137) had lymphocytopenia. Lung involvement was present in all cases, with most chest computed tomography scans showing lesions in multiple lung lobes, some of which were dense; ground-glass opacity co-existed with consolidation shadows or cord-like shadows. Given the lack of effective drugs, treatment focused on symptomatic and respiratory support. Immunoglobulin G was delivered to some critically ill patients according to their condition. Systemic corticosteroid treatment did not show significant benefits. Notably, early respiratory support facilitated disease recovery and improved prognosis. The risk of death was primarily associated with age, underlying chronic diseases, and median interval from the appearance of initial symptoms to dyspnea.@*Conclusions@#The majority of patients with 2019-nCoV coronavirus pneumonia present with fever as the first symptom, and most of them still showed typical manifestations of viral pneumonia on chest imaging. Middle-aged and elderly patients with underlying comorbidities are susceptible to respiratory failure and may have a poorer prognosis.

ABSTRACT

Objective:To understand the current status and main models of transformation of medical patent in the World-class universities in China, carry out research on the transformation of scientific and technological achievements based on the perspective of patents.Methods:The registration data of assigned medical patent of 42 universities were collected to analyze the overall status of transformation, the cycle and the transfer flow of patent transfer. The main fields and themes involved in the transfer of patented technology are shown by cluster analysis methods. The prominent transfer mode of medical patents in the universities was discovered by analyzing the transferees of assigned patents.Results:Among 42 universities, medical patents transferred within ten years account for approximately 6% of authorized patents, and the average transfer cycle is 28 months, and transferees are business-oriented. The transferred patents cover more topics including hydantoin derivative, fucoidan sulfate, human papilloma virus, brain-computer interface and minimally invasive surgery. 88% of patents are based on the traditional direct patent technology transfer model. In addition, it also includes transfer modes such as transfer through the asset operating company, the spin-offs, the third-party intermediary, the institute, and the technology transfer center.Conclusions:The overall task of building a World-class university is heavier and the demonstration effect is more obvious. However, the status quo is that the proportion of transfer patents of the World-class university is still low, the transfer cycle is long, and the mode of transfer of scientific and technological achievements is relatively single and should be more innovative. The World-class universities should improve the quality of their own scientific and technological achievements, improve the results transformation mechanism, and constantly explore the mode of achievement transformation in a more tailored way.

ABSTRACT

Perimetry is an important tool for diagnosing and managing ophthalmic disease, and it is uniquely important for predicting prognosis, monitoring therapy, and assessing outcomes in diverse contexts.However, children have a shorter attention span and are less cooperative and more easily distracted than adults, making it difficult to perform typical adult-based tests and obtain reliable results.This is a very serious challenge.So far, many methods have been used for children internationally, for example, the kinetic perimetry, and static perimetry included in adult-based tests, in addition to, new detection strategies and new examination technologies speciafically designed for children who are unable to perform for standard perimetry measures.At present, improving perimetry in children requires their cooperation, e.g., maintaining a continuous fixation on a central target during the test, reducing inspection time, etc.so that recognition of a visual field deficit, leading to early treatment, may possibly prevent subsequent vision loss.Different examination methods, algorithms and normative databases can be useful for testing normal children, infants, or children with cognitive impairment to ensure the most reliable testing possible.Through a literature review, this paper summarizes the various methods of pediatric perimeters.Critical expert analysis of the advantages and disadvantages of these methods is also provided.This will help both clinicians and researchers improve the accuracy of examination and obtain reliable results.It will also encourage other researchers to conduct further study on pediatric perimeters with a view toward making new breakthroughs.

Objective:Application Kano model to analyze the nursing service demand of outpatients and promote the improvement of outpatient nursing service quality.Methods:Design a questionnaire for the outpatients’service requirements based on the model of Kano, including 4 parts, service and mental outlook, management and professional duties, health education guidance, outpatient management and convenience Services, composed of 28 items. Using Kano model analysis technology, we can determine the different attributes of outpatient nursing needs, including O expectation attribute, M essential attribute, A charm attribute, R opposite attribute, and Q question answer.Results:The nursing service needs of outpatients were classified by identifying the attributes of each demand. There are 8 items1, 10, 11, 15, 17, 18, 21, 22 attributed to the desired attribute (O). There are 11 items 2, 8, 9, 12, 13, 14, 23, 24, 26, 27, 28 attributed to the mandatory attribute (M). There are9 items 3, 4, 5, 6, 7, 16, 19, 20, 25, 28 attributed to the charm attribute (A) . At the same time, the quadrant diagram of satisfaction and importance of each item is constructed.Conclusion:Using the Kano model to analyze the nursing service demand of outpatients can determine the attributes of each demand, realize the priority of management project improvement, and provide decision-making reference for improving the quality of outpatient nursing service.

ABSTRACT

Objective To investigate the early clinical characteristics and diagnosis of and therapeutic approaches for coronavirus disease 2019 (COVID-19). Methods The clinical manifestations and treatment of 62 patients with COVID-19 admitted to Hainan Province People's Hospital during the period from January 21 to February 10, 2019 were analyzed retrospectively. The COVID-19 patients were classified into 3 groups: general, severe and critical types according to the disease severity and the white blood cell, neutrophil and lymphocyte counts were compared between the 3 groups. The 62 patients were given symptomatic and supportive, antiviral, hormonal and Chinese herbal treatment. Results 79.03% (49/62) of patients had a history of traveling or living in Wuhan. The incubation period was (6.85±4.45) d. The clinical features including fever (80.65%), dry cough (62.9%), fatigue (25.81%), shortness of breath (19.35%), and dyspnea (14.52%), sore throat (9.68%), nasal congestion and runny nose (9.68%), muscle soreness (8.06%), diarrhea, vomiting (3.23%), and nausea (4.84%). 95.16% (59/62) of patients had normal or relatively low white blood cell counts. There were statistically significant differences in the counts of leukocytes, neutrophils, and lymphocytes between the 3 groups of the patients (P<0.05). 77.78% (7/9) of the critically ill patients with lymphocyte counts below 0.8×109/L. 90.32% (56/62) of patients typically presented with single or multiple lung lobes/ segments showing ground glass shadow. The average time for CT and CR showing progress and starting to improve and absorb was (9.05±3.75) d and (14.94±7.32) d, respectively. The total course of disease among the 57 discharged patients was 9-38 d, with an average of (21.05±7.17) d. The length of hospital stay was 8-34 d, with an average hospital stay of (15.32±6.49) d. After treatment, the time of the negative mutation of the novel coronavirus was (11.56±6.56) d. Obvious dyspnea occurred in 9 critically ill patients, among them 7 patients needed invasive ventilator support and 2 non-invasive ventilator support. Conclusions In the early clinical stage of COVID-19, fever is the main manifestation. Individual patients are asymptomatic. CT is characterized by ground glass-like changes. Critical patients have significant dyspnea, which can be accompanied by kidney damage. If lung imaging suggests a rapid progress of inflammatory exudation, CRRT (continuous renal replacement therapy) and respiratory support therapy can be given on an individual basis.

ABSTRACT

Objective:To systematically investigate the chemical constituents of 90% ethanol extract of Corydalis impatiens,and evaluate their inhibitory effect on cell proliferation in vitro. Method:The chemical constituents were isolated and purified by silica gel,Sephadex LH-20,ODS column chromatography and semi-preparative high performance liquid chromatography (HPLC).the structures were identified by spectroscopy methods such as NMR and MS,as well as analysis of physicochemical properties and/or comparison with literature data,and the inhibitory activities of 13 compounds on HepG2 and SMMC-7721 cells were measured by methyl thiazolyl tetrazolium (MTT) method. Result:Thirteen known compounds were isolated and identified from 90% ethanol extract of Corydalis impatiens as ethyl-5-hydroxy-2-pyridinecarboxylate(1),coryhumolide(2),3,4-cis-3,4-dihydroxy-β-ionone(3),megastigmane(4),9-hydroxy-4,7-megastigmadien-3-one(5),blumenol A(6),indole-3-carboxy acid(7),1-methyl-[1,2,4]triazolo[4,3-b][1,2,4]triazin-7-one (8),adenine(9),nicotinamide(10),2-hydroxymethyl-5-pyridinol(11),adenosine(12), and β-daucosterol(13). Cytotoxicities assay showed that the IC50 value of compound 3 for the hepatic cell line HepG2 was 24.7 μmol·L-1 (positive control drug cisplatin:4.8 μmol·L-1),and IC50 value of compound 4 for the hepatic cell line SMMC-7721 was 13.8 μmol·L-1(positive control drug cisplatin:5.4 μmol·L-1). Conclusion:Compound 1 was a new natural compound,compounds 3-8 were obtained from genus Corydalis for the first time,and compounds 2,9-12 were isolated from this plant for the first time. Compound 3 exhibited weak inhibitory effect on hepatic cell line HepG2,and compound 4 exhibited moderate inhibitory effect on hepatic cell line SMMC-7721.The other compounds rest of ones exhibited no obvious inhibitory effect on hepatic cell line HepG2 or SMMC-7721.
